Packaged Bank Account query!

Morning all
It has come to light I had a packaged bank account with Lloyd’s from a long time ago . I was also in a DMP and they continued to put this charge on every month also during me paying this off .
I don’t have the account number any more Cani still claim all charges back and any interest and how do I go about it please ?
