i have just received a General Form of Judgement or Order from the courts, what does this mean? has anyone else received one, what do i do, and what happens next?

i need to file at court and serve on the defendant an amended particulars of claim which must contain:
legal basis for claim
concise statement of the facts
shall specify- the accounts of which charges have wrongly been levied
the nature of the charges
set out schedule form of the charges, showing date
amount and description of the charge0
