hya my wife has both physical health and mental health issues and has just received the above form , where is the best place for help filling it in , she is capable of going downhill very quickly mentally and we need any help fixing it in , she suffers from chronic depression anxiety related issues and has previously tried to commit suicide and I'm afraid that all this pressure will bring all her fears back . as i said need someones help in going through it all

That is your best bet but you need to be quick about contacting them so that you can return the form by the due date. Just a thought - you could try contacting charities that deal with mental health they may have advisors. It's always hard contacting CAB in my area.
