Yes, they would need to know. What makes you think they shouldn't be told?
You can do permitted work when claiming ESA. This will allow you to try working to see if you can cope. If you can't then you've not lost anything. You can earn up to £125.50 per week and work less than 16 hours per week but you need to tell DWP you're doing this.
