Tax Credit claim questions

I have used the entitledto website which suggests I should be entitled to tax credits.
I'm guessing the actual application process will ask similar questions and wanted to check the following:
Amount paid for childcare - Is this the full amount based on my invoice or just what I pay after I've used the childcare vouchers? (eg. If the cost was £500 per month and £486 was paid using vouchers, would I put £14 or £500 a month?)
Earnings before tax and national insurance - Do I put the gross amount minus childcare vouchers and pension contribution, which are both deducted pre tax?
Pension contributions - Do I only put these in if they are paid after tax?

You need to check what's in your area, if it's a full universal credit area and you don't have more than 2 children then it's UC you need to claim and not tax credits.
