Ticket machines not working

I have received a parking fine from Parking Eye, stating that I was in the car park for 51 mins, It was at North Tees Hospital, where I had an appointment. You have 20 mins free and it would have been 18 mins when I would have left. I wasn't sure if I still needed to get a ticket, but the machine wasn't working. I am disabled and by the time I walked along to the next machine I was over the 20 mins. But that machine wasn't working either. I went back to the car and drove round to the machine at the exit. That one also said Machine not in service and I have a photo of this one. I read and re-read the notice board for a contact phone number. I was unable to pay on a card as I didn't have one.
I am not very good at finding things on threads as to which template I should use to make a complaint. Can someone please tell me what I should be using?

Have you seen the thread that tells Newbies to read it first? There are templates in that thread.0

Did you appeal saying you are disabled and that you struggled to get from broken machine to broken machine, and that you have a photo, and did you attach it?
There are no templates to a complaint to an NHS Trust. Write it in your own words with your photo of the broken machine and your proof of disability and ask why their contractor is refusing to make 'reasonable adjustments' of time for disabled people under such circs?
If you have a rejection letter and POPLA code, that must be used within 32 days BUT exhaust the PALS* complaint first, this month.
North Tees NHS Trust should be ashamed.

I have only just received the notice. So, no, I haven't sent anything in yet. But thank you for your help. Should I just send it to the NHS Trust? Or send another one to Parking Eye as well?0 -
Send a complaint & photo of the sign, and proof of disability as well, to PALS at the Hospital (Google it, they can normally be emailed). No letters in the post; too slow.
And appeal online to Parking Eye saying the same thing and telling them you have complained to PALS and expect it cancelled. And upload to their web-pages, the proof of disability and proof of broken machine and make the appeal in your own words, explaining about struggling (or hobbling, or slowly walking/gasping for breath, depending on your disability) between the machines in a fruitless search for how to pay.
Mention the Equality Act and your LEGAL right to a 'reasonable adjustment' of time.
