I thought the nhs exemption was based on the actual tax-year income so say for months 1-11 you were on track to qualify and used the exemption then an extra unexpected earning in M12 took you over the threshold then in theory you had to pay back what you had used of the NHS exemption?

NHS certificate is valid for 7 months. It is valid until the expiry date.

If entitlement is no longer due, due to change of circumstances, then once the certificate expires it will not be renewed.

So if circumstances change and no longer entitled to a certificate in month 5 but certificate expires in month 8 then the certificate will remain valid until month 8 but will not be renewed.
