Martin spoke about this on his Radio 5 podcast yesterday.
I found this from a few years ago https://forums.moneysavingexpert.com/discussion/161938/financial-factsheet-brainstorm-help-required&page=2#topofpage but it appears it did not get as far as a template for an easy to fill in chart. Perhaps the list got to cumbersome.

Mine is a simple (ish!) Google Docs file that I share with my wife. It's broken down into subheadings and under each is a table of information summarising account information. Here's what I've currently got:
Current Accounts: Account Name; Sort Code and Account Number; Account Holder (me, wife, joint); Minimum monthly pay in; How it is funded (e.g. salary, standing order from another account)
Savings Accounts: Account Name; Sort Code and Account Number; Account Holder, how it is funded (if anything)
Credit Cards: Account Name, Credit Limit, Account Holder, Whether it's paid off in full or how much is paid off and when 0% ends
Utilities and Bills: Type (e.g. Gas and Electric, Mobile Phone, Council tax), Supplier, Typical monthly cost, Renewal date
Vehicles: Type of Bill (e.g. Insurance, MOT, Tax, Breakdown cover), Supplier, Annual Cost, Renewal date
Mortgage: Supplier, Interest Rate and fixed period end, Account Number, Monthly amount
There are a few other headings but they're getting increasingly specific to us, e.g. a table of passport numbers and expiry dates.
