Voluntary Termination; Help!

Hello,
I have three months left to pay on my PCP.
I am taking out a new contract next week with a vehicle leasing company. So I want to terminate with my current dealer. I have paid over 60% of my contract so understand from reading other theads I am eligible to take a VT?
What do I need to do/say to my current car dealer?
