Equifax cannot identify me

Hello,
I've moved to the UK 13 months ago. My current account is 12 mo old, and does show on Equifax. I also have a 11 old broadband account (which only shows on Equifax), and 2 small credit building loans (cashplus and credit improver), which also shows on Equifax. I am on the Electoral Roll since Sept 2017, and been on Credit Ladder for 1 yr.
I just got a credit building credit card, easy breezy (500 GBP) - because my Experian file was checked, and I am identified with Experian (also on gov.uk).
However, I cannot get a SIM-only contract with O2 because they check Equifax, and I am told they need "more info".
Aside from getting a SIM-only contract elsewhere (which I will do), what can I do to fix my situation with Equifax so that it doesn't jeopardize my future chances of borrowing despite me being responsible about it? How can they have enough info to pull my credit report out but still be unable to identify me?
That is to say:
1. who do I ask what the problem is, and
2. how do I do it?
