credit report and DRO incorrect who do I write to?

Hello, I have probably been lazy about this but have been on my credit reports. My DRO finished May 2014 so next year will be 6 years in May and will hopefully all clear however the dates are all wrong with some defaulted date August! so I need then to hopefully change from when I was initially given the date also my accounts all show amounts outstanding and defaulted - Am i right in thinking this should say settled or partially settled? They all just show debt amount..
Anyway I have written letter with a template I had found but I absolutely have no idea who to address these to? I have Halifax credit card, Santander credit card and bank and Lloyds bank account. I just want to make sure they get to the right people! Thanks for any advise or pointing in right direction!

Address your letters to the Data Controller, and send to the complaints address for each creditor.
