Do PIP assessors contact your GP/Doctor? what form do they send them?

When doing a PIP claim, sending back your PIP "how your health affects you" Questionaire,
1) Do the PIP always contact your GP Doctor?
or only sometimes?
2) and under what circumstances do they contact your GP?
(eg, if you didn't send much medical evidence, or your answers to each question wasn't really backed up with any reports for example?
3) What do the PIP questions ask the Doctor, on their form?
4) In your own case (or somebody you helped) then did the PIP assessors contact your GP?
5) and at 'which stage' do they contact them?
(before the face to face stage? or After the face to face stage?
6) When they write to the GP, what is the name of the form they send the GP?
(when I mean Doctor, it can also mean any other health profession, neurologist, etc)
answers to 1, 2, 3, 4, 5, 6 greatly appreciated

It is very unlikely that the DWP will contact anyone. It really is up to you to supply as much medical evidence as possible.0

Again, like yesterday i'm not sure why you're posting duplicate posts for.
If you've recently applied for PIP try to relax, i know it's difficult but all these questions will only make it worse.
Most people have assessments. Times vary hugely across the country to how long the process will take from returning a completed form. It depends on backlog in your area. If you have applied recently, expect an assessment.
They rarely contact anyone for evidence, the onus is on the claimant to make sure it's sent.0 -
DWP have contacted my gp for a report, and that's on top of my extensive evidence that i sent in - letters from specialists/nurse/gp/support worker. its my first pip application and my esa review is still in and not heard since april.0

There's no timescales to assessment times/decisions for PIP or ESA. Good luck for both.0 -
They contacted my sons GP for further info, as I had to sign a form before they could release any info to the DWP. I didn't mind as I had nothing to hide. I also sent in my sons EHCP and annual reviews, which I didn't have when I sent the form through. They then phoned me and I answered all their questions over the phone. They advised my son wouldn't need a f2f, which made sense to me.
