I am confused, who can help: deed poll change name

on deed poll form: LOC024:
statutory delaraion for a deed poll for a minor

I can find a friend to sign it,
my question is:
once my friend sign it, does he/we need take it o a solicitor to proof it?

if so does my friend need go to see the solicitor as well?

    Yes, they need to sign the form in front of the solicitor.

    Do you have agreement from your child's other parent or elave from the court to change the child's name?
