Santander Mortgage Declined

Hi,
After 3 weeks of waiting Santander declined our mortgage. Reason of the decision says

Head offive review Failed
FCO decline

What does this really mean? My MA doesnt give me a clear answer and I had to log in to intermediates to see this information.

    It means you wont be getting a mortgage with Santander - go back to your broker, or if you went direct go find a broker.

    What does this means?

    Your mortgage adviser should be able to get a clear reason on why you have been declined, whether this is down to credit score, affordability, failed criteria etc. Maybe worth asking again, he will need to get in touch with them. Maybe it is something simple that can be amended or your broker may need to see what's gone wrong and consider alternative lenders.

    It sounds like you have failed the lending criteria for Santander. Did you provide all the income evidence requested and do you meet affordability criteria? Income multiples and little to no other debt?
