Fillin in PPI Complaint with only an account reference

I originally asked the Claims Guys to search. They came back with 2 lenders references for PPI with the dates taken out.
For one of them I have no paperwork and the bank don't hold it either (written and asked).
Was taken out in 2001 and beleive it was a loan, but not sure. The claim form asks amongst other things:
1: Who's names are on policy.
2: Reason you borrowed the money.
Unless I find out what exactly it was the I can't answer. Suspect it may have been also in the name of my girlfriend at the time, but not sure.
Anyone have any advice?
P.s Will be making my own claims, without a midleman.
