PIP - have to hand-write form?

Hi all,

This is perhaps a really stupid question, but, I've typed out answers for each of the essay style questions, and the majority of them won't fit into the 15 or so lines given. Can I simply write "see page x" on each one and just include the printed document?

And also, I have around 12 pieces of medical evidence to include. Do you think it'd be alright to write a one page overview or the evidence included?

Thanks in advance,
Cal

Yes. Typing a response is a good idea.
Do include your name and NI no on each sheet.
On the form write "See attached sheet" on the relevant pages.
I'd include copies of all the medical evidence - again add your name, NI no, and firmly attach all sheets to the form.

It does state on the form (if I remeber somewhere at the beginning) that you can write extra information on a separate paper.

They never leave enough space to write all the relevant information. I did write on the form and typed the additional information on a separate sheet. There were many questions that I needed additional information, so what I did was, say for example Q3 - I would state next to Q3 see further information on separate sheet. On the separate sheet I would put 'Q3 additional information'- I added all extra info on that layout.

Don't forget to place your NI number and full name on top of each separate sheet. I also typed any other 'extra' information I thought was relevant on a separate piece of paper

I type all my answers. Put my name, date of birth and NI number at the top of every page and a page number at the bottom.

I clearly number each answer and print labels that say 'please see additional info page ' then stick these in the answer boxes and pen in the relevant page number.

I put a cover sheet with medical evidence that is basically an index of dr, 'job title/clinic name' and then run a hghlighter over the key bits.

save your types answers so you can use them as a starting point for renewals..

i have never understood why the form doesn't have a PDF version to type in, one theory was they didn't want people to feel pressured into writing too much, but the degree of info they want just wont fit in the boxes .. even typed and then cut and glued.

I hope it goes well for you and you get the right award

My PiP assessment was last year...

Tick boxes were done with a pen....and simple sentence answers were hand-written (by my Mum....with a note at the end to state that she had done this for me)...additional information was typed.

Each additional page had my name and NI number on, and I photocopied the entire form plus evidence sent ...for my own records.

Do you have any advice on how best to do this Alice? I am worried they'll lose the sheets, although like you all advised they have name and NI, highlighted in yellow.

Treasury tags or staples, will ensure it stays together.
    Keep a copy for yourself.
This may be useful re the medical assessment:

    Good luck.
