Hello - I am about to start a reclaim but as it was about 20 years ago the account was in my maiden name. Do I put that or my married name (ie my name now) in the forms? Also not sure which address I was at - is it advisable to put both addresses in the box asking for additional information? I think I may have moved a few times during the time I had the accounts.
