Whether these debts have come about through fraud or just poor judgment is not necessarily the main issue right now. You need to ensure that you are 100% in sole control of these accounts now and that any correspondence is coming directly to you - that means cancelling any cards and making sure that any post cannot be intercepted.
Either way you are likely to be held liable for what is owed, so please do as advised above so that the debts at least do not get any bigger.

How much was the loan for and what is the outstanding balance?

Has she been making repayments to you for the loan?
You have got yourself into a complete mess here. You say she has taken out credit cards in your name and has been using them - so you obviously knew about her doing this so just as complicit in the fraud as she is. If you go down the route of contacting the card companies and police and saying she took them out fraudulently using your ID, then be prepared to be hit with a CIFAS marker 3, 6 or 7 take your pick.0 - Protective Registration - Recorded at the request of the person named.
1 - False Identity Fraud - Use of a false name with an address.
2 - Victim of Impersonation - Use, by another person, of this name and/or address.
3 - Application Fraud (Facility Granted) - Use of name reasonably believed to be genuine, but with one or more material falsehoods in personal details or other relevant information -; the facility was granted.
4 - Application Fraud (Facility Refused) - Use of a name reasonably believed to be genuine, but with one or more material falsehoods in personal details or other relevant information -; the facility was refused.
5 - Conversion - Conversion (disposal or sale) of goods (to which the hirer/buyer does not have title) under a hire purchase, conditional sale, contract hire, leasing or rental agreement.
6 - First Party Fraud - Opening an account or other facility for a fraudulent purpose, or the fraudulent misuse of an account or facility.
7 - Aiding & Abetting - Aiding, abetting or assisting, or conspiring with, another or others to fraudulently procure credit, or other facilities, or hire products or services.
8 - Insurance Claims Fraud - The making of a claim(s) under one or more insurance policy (ies) with one or more material falsehoods or by presenting a false or forged document.
If you get hit with a CIFAS then wave goodbye to any mainstream credit, any bank accounts/credit cards you have being closed for the next 6 years.
The problem you'll find is where she has taken out the cards and I assume they were taken out years ago and payments have been made to them, if you try and cry fraud to the credit card companies, look at it from their point of view. You live at the same address, the cards are in your name so you would be opening the letters, monthly repayments have been made, card in regular use - why all of a sudden now are you citing that it wasn't you who took them out? You would have to prove that it wasn't you using the cards and if they ask your girlfriend all she has to do is deny everything, it would be her word against yours.
The way I see it you have 3 separate issues to tackle.
1. You want to break up with her
2. You want her to continue making loan repayments
3. You want her to stop using credit cards she took out in your name and clear any balances
Your choices are:
1. Split up
2. Talk to her and ask her to set up a monthly standing order for the loan repayment and then to give you all of her (your name) credit cards and a separate monthly standing for any balances outstanding until they are cleared
But I get the sense from your post that she won't give a monkeys in which case the following will happen:
1. Split up
2. You ask her to repay the loan, hand back credit cards and clear those balances and she tells you to foxtrot oscar and she's not paying a penny back of anything
3. Cry and then you can either look to starting a debt management programme with StepChange, go bankrupt, or make a budget and slowly clear the debts until everything is repaid and you learn the life lesson of never lending a penny to anyone ever again.
In the meantime, if you want to know exactly what cards she has taken out in your name you need to obtain copies of all 3 of your credit reports which can be done here for free:

There are 3 in my name one is mine but my credit score is in the red
All 3 are yours. The issue is whether 2 of them were opened fraudulently in your name, and without your knowledge. If you knew and did nothing about it, then you are complicit in the fraud.No free lunch, and no free laptop0
