Reconstructing uneven kitchen floor

We are planning to redo the kitchen of our new home, and as part of this we want to fix the kitchen floor, which is clearly sloping/ not level.
The current floor is 4m x 2m and tiled, and the survey we had indicated that there was unlikely to be a damp-proof membrane, so we'd likely have to have this put in as well as levelling the floor.
Has anyone previously had similar work done, and how much roughly would it cost?
