PIP claim form

Hi all.

I have sent my husbands PIP form back without taking a copy of it. I only have the Q1a Q1b etc written down and not what each question was! Does anyone have a copy of a blank form so I can see what Q1 - Q15 is.

Also would the person doing the assessment contacted his dr before his home appointment?

No, they very rarely contact anyone for any evidence. The onus is on the claimant to make sure they send any evidence to support a claim. Did you not send any evidence?
