PIP claim form

Hi all.
I have sent my husbands PIP form back without taking a copy of it. I only have the Q1a Q1b etc written down and not what each question was! Does anyone have a copy of a blank form so I can see what Q1 - Q15 is.
Also would the person doing the assessment contacted his dr before his home appointment?
