Filling of the post office uk third party closure form/step no.4!!

Hi to all forum mates,
I have with me Post office UK card Account Third Party Closure Form as my father who is a deceased now died outside the UK was maintaining his card account for his pension purposes.
I tried to contact the relevant post office by phone in Slough UK but still my question is still unanswered by their end so I am putting my question here in this forum.
My query is about the closure form step 4 Payment Instructions????
There i see Account Number , sort code, name of payee and underneath the residence address of the payee.
There is no such column indicating Bank Name where the money will be transferred so how will you come to know the bank name where which country the money is to be transferred???
There is no such column indication my bank IBAN for accurate transferring???
Please clarify where to write bank name? where to write IBAN in the form????
