PIP assessors report

Can anyone tell me what CQ, AR, FH and HOC stand for on an assessors report please

    CQ is claimant questionaire,
    HOC is history of condition
    AR..assessment report?
    I have no idea what FH is but others may know.

    Have you added up those points to see what you've scored? There will be boxes with dots in that report somewhere. You can use the PPIP descriptors or PIP self test to see what you scored.

    FH could be forensic history depending on the context (mental health and whether there is any history of offending).

    I added up the points and hopefully get standard care and mobility. The only thing that I find strange is I score no points for planning/following a journey were as I got 4 points last time and nothing has changed. I worry if i ask for a mandatory reconsideration that the whole thing could be lost

    FH- functional history ( basically the answers you gave to questions during the assessment)
