Just to add there aren't any 'MR forms' you just put it in writing that you wish to put in one and your reasons why, then add in any further evidence you may or may not have and take it from there.

I had to do an MR many years ago and I rang first to inform them of my intent and advised further evidence is in the post so can they not make a decision until that arrives.
I did all this within five days of the decision as I was given it over the phone before my original award notice was even received, yet surprise surprise within one day they had turned me down for MR with carbon copy of the first decision!
Only standard mobility I was awarded so I was advised by the DWP decision maker at the time to put in for an 'unplanned intervention' due to me not having my further evidence looked at and sure enough they wanted to do yet another home assessment but this time I had a nice paramedic man who wasn't there long and within a week I had gone to enhanced both and still have that award now four years on!
I was reassessed last year again at home, and given further three years.
I do think it's ridiculous to have to keep re applying every couple years, especially when your condition is life long and no change is expected!
There never used to be but there actually is forms now. It's a new thing that came out mid March 2018. It can be downloaded online.

    Hi,

    Thanks for all your support.
They won't award me pip. all lies again on what they tested me for.

So the next step is appeal to tribunal. sounds scary.
It says i can appeal in writing.
Can someone who has been down this route, please advice what to do next please, and would i have to go to court?
what would i even write ?
I can also get a letter from my doctor, who also said if they don't award me, he will deal with it. not sure if that's true?

    Have nice weekend.
    Thanks again.

    Regards
    Mark.

    Mark,

Go back to my post at 7.
Have you looked at this guide:
    https://www.advicenow.org.uk/guides/how-win-pip-appeal
This tells you how to appeal.

This is the form you need to lodge your appeal:
    https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/695156/sscs1-eng.pdf
You need to enclose the 2nd copy of your MRN.

Do you have a local CAB / advice centre who can help you?

Here are some useful online resources:
    https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/appeals/apply-to-tribunal/
    https://www.rethink.org/resources/p/personal-independence-payment-pip
    https://www.benefitsandwork.co.uk/personal-independence-payment-pip/pip-appeals

    Good luck.
More than 65% of PIP appeals succeed.

I would strongly advise you to attend the tribunal hearing, you can explain the difficulties you experience, and clarify any disputed points. You verbal evidence is very important to the tribunal.

    Hi guys.
I got a call from DWP today offering me £108 a week so not to go to court.
Or i can go to court if i don't accept it.
I got doctors letter and another one, which seemed to help
You think that's ok to accept it?

    Thanks for all your help.

    Mark,

    So, have they given you:
PIP Daily Living Enhanced Rate...... £85.60, and
    PIP Mobility Standard Rate.............. £22.65

    That looks reasonable.

Only you can decide if you should have been awarded:
    PIP Mobility Enhanced Rate............ £59.75

And if it's worth continuing the appeal.

Be aware that the tribunal will look at the entirety of your award.

Why would you not accept the offer? Once that award is in place there is nothing stopping you appealing against the award - the new decision carries the right to appeal..
