PPI claim

I have contacted my local Building Society with regard to an Endowment Mortage I had with them and mis-sold PPI. They confirmed over the phone they had my mortage details and would be in touch. I have now received a letter containing my mortage number and a PPI consumer questionnaire asking various questions to complete. They should already have the answers to all these questions has they have confirmed they have my mortage information on record. Should I fill in this questionnaire which isn’t their original paperwork but a copy of the Financial Ombudsman paperwork?

Sorry I haven!!!8217;t explained myself very well, I haven't an issue with completing a questionnaire from my building society and signing to say I want them to investigate but this questionnaire at the end asks my husband and I to both sign stating !!!8220;We confirm we want to make a formal complaint about the sale of the payment protection insurance described in this questionnaire!!!8221; for the financial ombudsman. I have asked my building society to investigate my PPI and I was under the impression they needed to make a decision if I was due PPI or not and then if I received a no it was then my choice to continue and persue the matter with the Financial Ombudsman?0

The form is for you to fill in and then send back to the bank, they do not send it to the Ombudsman.
You make the claim so they need all your details from you.
Make a copy in case you want to go the Ombudsman.0 -
