I would just put in your own words what happened and why you feel it is unfair.
Mention your disability and how that affects you. (Set the scene).
Have a go and post it here for review.
I would include a copy of your blue badge and/or other evidence of your disability.
I also suggest a letter of complaint to CEO of any store you visited particularly if you have a receipt for the day (send a copy with it).
