Britannia Loan PPI

Can anyone point me in the right direction?
I took out a loan with Britannia building Society (not a mortgage) it has PPI on it and I assumed I would claim the the Co-operative Bank who took them over, but I’m redirected to a downloadable form on the FOS dated 2012. Resolver doesn’t list Britannia.
Has anyone claimed on a Britannia loan?

I’ve answered my own question, I have to send the form (which doesn’t download on an iPad) back to the Co-operative bank.0
