Birth Certificate Query

I'm registering my daughter's birth tomorrow and her father has now decided he wants to be on her birth certificate. I have found the form that he needs tofill in (Form 16) to say that he's the father. (He can't attend to sign himself) I'm a bit confused as to who is able to witness a signature though on it. Any ideas?
