    Wing Commander and the such are my favourites. The insurance websites usually have a great list. I used to work in abbey insurance on the it systems and the database was massive.

    I have been known to complain to websites that include captain and lieutenant but omit flight lieutenant... That said, I note even the BA list leaves out the lowly pilot officer; perhaps they are either too rare these days, or aren't considered pecunious enough for an Executive Club. I also noticed that sgts and cpls are catered for, but warrant officers are also omitted. Brave, BA, very brave...

    Depends what page you read
    http://en.wikipedia.org/wiki/Sultana_(title)
    says only appropriate to use it for some now deceased rulers.

    Not on BA list so they must assume no one has that title.
