New claim for tax credits

I am due to have my baby in February and will be entitled to tax credits once baby is born.
I’ve requested the form but I’m confused as to what p60 I give? Does it go off my 15/16 or 16/17 p60? I will be starting my claim the day my child is born, which is going to be sooner than I expected as I’m having a planned c section. So I’ll be able to start claiming from next month when my child is born.

Last year’s income, 2016/17. It should tell you this on the claim form.0
