PIP assesor report and DWP report different

Hi, just looking some advice and if this has happened to anyone else.
I got my mandatory reconsideration letter, stating the same as first letter from DWP, i wasnt awarded either part of PIP. So i asked to see the reports thinking the assessor hadnt listened to myself or my mother at all. In fact she had. She had ticked 'needs prompting or supervision' with a lot of notes underneath each question.
The report done at DWP on 2nd page says, customer has scored 13 points on DL and 8 points on mobility, please advise if this is consistent with current scores.
The DWP report has ticked nearly every box as 'can do this unaided'.
They awarded 3 points for DL and 4 points for Mob. So not entitled.
Has this happened anyone else? Has DWP gone against what the assessor who seen and listened to me awarded?

Yes, I have seen (at my local CAB) DWP decisions overturning the points awarded by HCP.
However, I have not encountered a case where an enhanced award was reduced to no award.
In most (if not all) cases the original award was re-instated at appeal.
Is there any rationale for the DWP DM to overturn the HCP's report?

I would suggest getting evidence to support your mother's appeal (GP, carers, family members, etc)
See if your local CAB can help with the appeal.

thanks. It was my claim not my mothers. They just kept saying not enough evidence. ive appealed to tribunal now. I guess I will see what happens there. Just was really strange to see those points then be awarded nothing0
