As you don't know him well enough to pick something that is definitely suitable, go with the neutral - gift card for somewhere that pretty much anyone would use. John Lewis, Amazon, etc.

A lot of suggestions could easily fall foul.

Subscription to Men's Health? Maybe he's already got one.
T-shirt? He's probably got enough T-shirts.
Book? Maybe he's already got it or doesn't read much.

A card with cash would probably be most appreciated.

Wow, aftershave is very personal, more like what a partner would give than just a client, hope he likes it & doesn't have it. I thought you had a normal PT/client relationship hence suggesting general things
