No, even small purchases are zero vat rated if they're aids for someone who's disabled.

If you're in receipt of the enhanced element for mobility, you could consider swapping the allowance for a Motability car. You could also ask them about being assessed for possible adaptations to the car.
The Disabled Adult Railcard is £20.00 per year. I've not heard anything about being upgraded to first class for free, though. I always book it anyway so there is more room for my mobility aids, it's less crowded and there is room to stretch/stand up when I need it. When you book your tickets you can also book special assistance, eg have someone to push your wheelchair on board, or just carry your bags on board, help you get seated etc.
If you're planning on flying anywhere, you can also book special assistance to help as needed, eg a wheelchair to get you to the gate, an access lift if the plane is off stand and you can't climb stairs.
I'm in Scotland and my bus pass is more than just a simple bus pass. I can use it on trains in the local area and get a bigger discount than even with my railcard. It also allows me to have a companion at my local cinema at no charge. It's a Showcase Cinema, and I haven't tried it in any other chains yet, but I imagine it'll be the same. Check out if your bus pass has any alternative uses.
I just bought my Radar key from Amazon for a couple of quid. Sadly, you don't need to prove anything for this, so I guess anyone could buy one and get access to the locked loos!
If there are any concerts/events you'd like to attend, find the Disabled Services (or similar name) department for the venue and give them a call to book tickets. Usually, your companion/carer will go free.
