Questions around Application to file a Statutory Declaration Out of Time - Form PE2

Hello,

So the back story so far..

I have received a letter (first knowledge of offence) through the door, apparently hand delivered by Bailiffs (Newlyn PLC) seeking take control of goods to the tune of £513 on the basis of an unpaid fine for a Bus lane Charge. With my correct details.

Initially I thought some kind of con, so rang the council to confirm the fine actually was ever issued. Turns out it was real, issued way back in Feb to my previous address in Newcastle.

I was made redundant in Newcastle and found work in Enfield, but was trying to sort some accommodation down there, when the fine must have been sent. Wound up crashing on couches for awhile as the work contract fell through, essentially homeless and couch surfing, I requested to my kind friends I was lodging with could I reregister the van there, so as to sell it, So I did briefly - whilst selling.
Anyway so now the people i was lodging with have forwarded this letter on to me, very embarrassing and hard to explain how you're going to make that disappear when they know you've no money.


So anyway.. nightmare. So the fine is real, now with bailiffs, kind woman on the phone at the council (i know) said they sent many letter before it was passed to bailiffs, then a 'not at this address' was received, so they know that, then the bailiffs must have dug up the last registered and found that house, why the council couldn't, another matter.

She said I could file a "Late statutory declaration form" to get it back in the councils hands.
To contact Northampton County Court with a provided number.
I've spent (no exaggeration) 30 mins just ringing out, not on hold, just a ringtone, it cuts off after 7mins always and then called back tried this 5 times.

So... to the point:

Now I've found the form online (I think - linked below) but she said I'd have to have it witnessed and to make an appointment with my local county court.
- Can I print the form myself?
- Is there anyone a little easier that can witness as I fear waiting for a court appointment to sign, then posting etc. will delay - whats a very stressful situation,
- should I explain this to the bailiffs?
- Should I mention that isn't my address? But then they will want my current which isn't any more mine than the last either.

Any help would be gratefully appreciated, I'm a little flustered after a hard year being pushed around by life, I don't have £13 let alone £513 they're asking. The original fine I'd need monthly payments to make. Last I need is trying to replace someone who tried to help me's possessions.

    Go to pepipoo.com and sign up there. Council ticket forum
    You will need dates and copies of the paperwork you DO have, redacted of ame, address and VRM, plus any representations made

    These forms are HIGHLY tetchnical. You MUST know how to fill them in properly, and it is non obvious.
