Specified Adult Child Care

Options
I have recently read an article in Saga magazine, which interested me.
My husband gave up work when he was 55 for personal reasons. He has never claimed any benefits as it was his choice. He is 64 now and recently had a government pension forecast. He does not have his full quota of contributions.
On reading this article which was entitled Grandparents claim for childminding, I realised that he would be entitled to claim NI contributions for the time that he looked after our Granddaughter which enabled our daughter to go back to work.
I found the link on the government HMRC site, have completed the forms.
My problem is, I cannot find the address to send it to. I have looked everywhere, tried to phone the number given but recorded questions do not fit my problem.
Does anyone know where I should send it to?
Keep on trucking!
