Same here, tried refreshing, still not working.

In Chrome, there'll be a shield in the top-right hand corner; if you click it you'll be given the option to load unsafe scripts.

Doing so will then bring up the form, but with a warning that the information isn't being sent over https, so it's not secure.

Therefore it's up to you to decide whether it's worth risking your personal info!
