Not knowing when ESA Review date is

I understand. But "to be on the safe side", you should also apply for a home visit in this way, incase they decide to require a face to face.

What is the best way to achieve this again? I guess I am trying to understand why they have done this both times we've received an ESA50 and make sure we have this again.

Just do the same as you have done before, with the added bit about a home visit application should a face to face be required. I don't know the situation, but maybe the doctor would write that even with a home visit environment he is likely to "kick off" so to speak.

Also, next time he is assessed we are hoping that, under the new change of policy, they will decide that they do not need to see him again. What is the best way of a achieving this?

Ask the doctor to confirm if he thinks this is a life long condition unlikely to change.
