couple of quick questions before I post MR appeal

1. Is it correct for the appeal to be written in the first person?
eg if the claimant is John does the appeal need to be written like "I have x and need help with y" rather than "John has x and needs help with y" even if the appeal is basically written by John's family/carers?
2. Address to send it to, it is definitely this address isn't it?
