Has anyone received a pip4 form and thought "what even is this?".
Mine is complete nonsense, short statements that contradict each other and are completely irrelevant to the claim anyway.
An example:

I'm a rural girl, I was driving tractors at 10! Of course I can drive I'm just not allowed to, they have a letter from the DVLA proving this. And even if I was allowed to drive what does that have to do with the price of cheese, there isn't a driving descriptor.
And the whole report is littered with spelling mistakes, their instead of there, lfit instead of lift, aren't these things supposed to be quality checked?

does not drive. Never learned. Refused application. Can drive husbands car on private land. Medically disqualified
And also:
has no warning. Has some warning. Pressure over nose and light headed. Happens rarely.
Can experience several in a day. Has 4-5 per week. Is disorientatred (sp) after. Needs sleep. Forgets things. [snip] has no cognitive issues. Was smiling during assessment. Said goodbye

If the PIP 4 you talk of is the report from the PIP assessor, then both of my assessors must have been almost completely illiterate. Serious schoolboy spelling mistakes, no grammar to speak of. If the assessors don't have even basic English skills what chance does the DM have.
They are quality checked by people of the same sort of grade as the original assessor so the spelling mistakes will, sadly, be left as they were written originally.Master Apothecary Faranell replied, “I assure you, overseer, the Royal Apothecary Society dearly wishes to make up for the tragic misguidance which ended so many lives. All the best drivers learned in tractors :-) I vividly remember my friend's dad saying 'ok take your foot off the clutch ... ... ... slowly' as the tractor tipped backwards!
I think the reports are part written prior to the assessment with the liekly options pre-added and then the wrong answers deleted (or not). That seemed to be what happened in mine. There was text already there and most of it was deleted and retyped after I answered0 -

I'm afraid that really doesn't surprise me.
The standard of some assessments are appalling. A male claimant's report stated '(S)/he can fasten her bra from the front........"!!!
One report dismissed problems with social engagement because the claimant had "good rapport with his dog..."!!!
Etc, etc, etc - and the numerous spelling / grammatical errors can be very entertaining (if I can use that word in this context).
The great advantage of that type of report is that it loses any credibility when placed before a tribunal.
I am always happy to receive a DWP evidence bundle with such errors at my local CAB, because I know that given a reasonable submission from us, medical evidence from GP etc, and clear verbal evidence from the appellant at the hearing will inevitably result in a successful appeal.
I am though extremely annoyed by-
the sheer waste of public money caused by this level of incompetence,
the distress it visits upon our clients,

I apparently brushed my long hair out of my eyes - used to 'prove' I have full motion of my arms.....( in reality, I have female-pattern baldness and keep what hair I have closely cropped!)
I was observed to be rocking constantly, crying at times, shaking, and constantly looking to my mother/carer for reassurance....and yet I also showed no signs of anxiety and was calm throughout!
I gave the assessor a printed list of my prescribed medications (it was from the chemist)...and they STILL managed to misspell the medications and get the doses wrong on the report.
Oh, and my assessor is a mind-reader who can communicate telepathically! ...what other explanation could there be?...The report includes details of questions asked during the mental-state part of the assessment...and records answers I gave....Not only do I have no memory of being asked the questions, and answering....but my mother/carer who was with me throughout is also convinced the questions and answers never took place!
Oh...and whilst it was stated (correctly) that I cannot walk without my wheeled walker ....I also somehow manage to walk down the road unaided, including crossing the street???0 -
The DWP have cured autism! I'm no longer autistic because I said goodbye to the asessor!
Oh and I stand to shower! Who comes up with this!0
