I understand that 1.e) is no longer awarded if someone cannot follow a journey at all because it would cause them psychological distress, but what if the person was vulnerable and actually at risk of harm if they were to go out in public by themselves without any assistance at all?

Presumably if they were vulnerable and at risk of harm if going out alone, they would score 12 points as being unable to follow a familiar journey without another person.
My son has Down's Syndrome and cannot go out alone. He has no road sense, or stranger danger sense, despite working on both for many years. He cannot find his way to my mum's house in the next street. Although he has not transferred yet from DLA to PIP, I expect him to receive twelve points for this, as he is unable to go out alone, ever.
Tasks should be carried out safely, to a reasonable standard, as often as necessary, and in no more than twice the time is takes others. In your OP, the important thing to note is vulnerability - so the person cannot carry out the task safely.0 -
I am clinically advised not to go out alone, for safety reasons. I submitted evidence of this, and have recently been awarded 12 points for PIP mobility, so it is certainly possible to get the points for this.0

No that's not correct - 1B and 1E are now the only descriptors that can be awarded for psychological distress. It is 1D and 1F that are not available for reasons related to psychological distress.
