Self Employed Sick note

I've just been signed off from self employment by my GP who said that I would need to get a form online to get my sickness benefits.

But there's thousands of forms and I am in agony and just can't think straight, can anyone help me and give me the link for the form?

    Depending on your circumstances and location you could be claiming ESA or Universal Credit. Search for either on the gov.uk site (don't use others as they may charge). You actually make the claim by phone, not by completing a form.

    Sorry I should have said that I need the sick note for my first week off that I have to fill in.

  • Are you a sole proprietor or a limited company? If the latter you would need to claim SSP from the company. The former would claim ESA/ UC as above.

    You don't need a form for your first 7 days sick note. It is called a "self cert" and the DWP automatically allow you to have the first 7 days as "a self cert" without anything from you except the fact that you have asked for your claim to start from xx/xx/2017

    There is actually a form for this, but I have never seen anyone use one, it is an SC2

    Do you have savings over £16,000 ?
    Or do you have a partner who works 24 hours or more?

    If yes to either of these...then you will want to ask for ESA Conts only (if it is ESA postcode)
    or
    ESA Conts New Style (if it is a Universal Credit postcode)

    **and important**
    Make sure your National Insurance payments are all fully paid upto April 2016. BEFORE YOU MAKE THE CLAIM.

    In which case:

    If ESA postcode..
    ask for ESA IR

    If Universal Credit postcode
    ask for Universal Credit and ESA Conts New Style.
