Declaring Bank Account interest on a cash basis

Hello, can someone help me with how to fill in my self- assesment tax return? I hope I'm posting in the right sub-forum.

I'm declaring my small income on a cash basis. I have a number of bank and building society accounts that pay interest. Two of these I started during the year I'm reporting.

Do I just declare the amount of interest shown on the annual tax statements from the banks, or do I go through my monthly statements and note any interest paid?
The two accounts I started within HM Govs tax year, which strangely is the same as mine(!), haven't provided relevant tax statements.

Also do I declare cash back as income?

    Any interest paid between 6th April 2016 and 5th April 2017 which will be shown on end of year interest statements.

    No cashback isn't income (although certain kinds of rewards like Halifax/Coop £3/£4/£5 per month is income as that's not classed as cashback although Barclays rewards are cashback).
