My neighbour used this below when her esa form went missing
For future reference
https://www.gov.uk/government/publications/employment-and-support-allowance-claim-form
Printed it off filled it in,wrote duplicate on it and sent it data post and know in esa sg.Advice given on Assured and Regulated Tenancy, Further advice should always be sought from a Solicitor....0 -

This is good if you are claiming for the first time, but the essa50 form is different. I think you can download this form somewhere, but it is a long one to do and print.0
