Working tax credit new claim

Hi my daughter who is on high care DLA is filling out a new WTC claim form. She is going to be working 16 hours self employed.
The form doesn't ask a great deal. I can't even see anywhere it asks for proof of her disability, or her projected earnings for next year, or even what she is doing for self employment.
Do we attach extra information, or do they write and ask for more?

Being on DLA is proof she is disabled.
Once the claim is processed she can notify them of any changes to this year's income.
They will ask if they want to know more about her self employment.0 -

Just fill out the form, they will ask if they need more information. They can check DLA records.0
