Swimming in the sea would be covered as standard as long as you stay in the marked swimming areas. They don't always have marked areas in lakes, so as long as a lifeguard was present that would still be fine.
The definition is not currently in our policy wordings, but I am in discussions with our Underwriters to get this added. I am also going to amend the activity packs on our website to include this information.

Not according to your policy. No mention of marked areas or lifeguards.
Activity Pack 1 covers swimming (pool - not open water) and swimming with dolphins.
Don't just make up what is covered.0 -

That is going to be changed.
I've double checked with our Underwriters and the intention is to cover swimming as standard, unless it is in open water (i.e. outside of marked areas and/or without a lifeguard).

When the op mentions Holidaysafe being recommended by MSE that does NOT refer to the MSE forum (as a read through the posts thrown up by a forum Search shows)
And MSE recommendation is based on price. It is in the guide to cheap insurance. Also note the significance of the asterisk next to the link in the MSE guide!0 -

Its a reasonably priced annual policy. There will be exclusions, another one is no cover for lost or stolen passports.
Imagine a family of four have passports stolen, it could be thousands to get documents to get home.
friend of mine lost his passport the morning we were flying home, He was allowed on the plane and simply had to fill in a declaration when we arrived home0 -
How about snorkelling and scuba diving? Are they only covered in marked areas with a lifeguard present (a rather important detail to leave out)? Or are they not a form of swimming - in which case I can presumably go swimming where I damn well like, so long as I take a snorkel with me??!!
I think some of your terms need a bit more thought, to be honest.0
