You're going to have to be a lot more specific to get any meaningful answers.
Are you looking for a paid carer or a volunteer? Is the person physically disabled/a wheelchair user? Have they had any sort of assessments? Are they in receipt of any services already? Can they afford to pay someone to go with them or are they looking for it to be funded?All shall be well, and all shall be well, and all manner of things shall be well.

I was therefore intrigued to see you had one response. I had some horrible images..lol.
Anyway I agree with above... how specialist does the person have to be (the monitoring aspect needs clarity)... what funds are available... are they involved in any services... what about social groups or even people closer to them... their disability is there any organisations locally linked to their disability to ask for advice if not support?"Do not attribute to conspiracy what can adequately be explained by incompetence" - rogerblack0 -

A wheelchair user limited funds. Does not go anywhere currently. Needs someone to keep an eye out and just keep company0 -
Best place to start would be a social work assessment, the adult disabilities team should be able to assess and see if the person is eligible for help to employ a PA.
Does the person want to go out and if so do they know where they want to go? They can share costs with another disabled person if they manage to come to an arrangement with each other. Myself and 3 others share the cost of a care company collecting and dropping us at a tennis group and then taking us home and they send a care assistant help with tying down the chairs and working the ramp. It's much cheaper than everyone getting taxis and hiring people to help in the tennis club.
That sort of thing can be arranged by approaching private care companies and asking if they have anyone looking to go to (for example) Asda every fortnight on a Thursday with tea after.
If they are sporty google "wheelchair sports in [area]", the proper groups will usually have transport help available but if the person gets into it then the costs can mount up for chairs and equipment though there's usually grants and things available.0 -

They are volunteers and have been very helpful with my husband, you may have to be on the list for a bit till they can find someone to help but it was worth waiting for.0
