What is the worst or daftest fault you have experienced in a game?

This is inspired by an experience while playing assassins creed syndicate recently. There is a mission where Karl Marx is makiing a speech to a crowd and you have to kidnap four specific people from the crowd without any of the other of the four seeing you do it. So I got the first two no problem leaving two to do. The problem I faced was that they were stood looking at each other. Now how exactly was I supposed to kidnap one without the other seeing. I don't know what was going wrong but after several fails the game righted itself because when I finally did it the final two ran away in seperate directions instead of standing looking at each other and I just had to run after each of them and tackle them to the ground.

    Got to the end of the original Prince of Persia on one of my many many playthroughs and the Grand Vizier Jaffar just stood there and didn't draw his sword. Made for an easy (too easy) end.

    I can't think of any specific examples but the most annoying one imo is where the bug prevents you from completing a mission/quest.

    Witcher 3 seems to have quite a few. Including one which spawns a high level leshen when doing a low level quest (quest is in a cave, supposedly the leshen patrols the woods but for some reason since a patch, has appeared in the cave also). Most of them right themselves if you reload but that one, you need to wait for his patrol path to be out of the way or he'll spawn in behind you.

    If I remember right, AC have a habit of having glitches. I recall in earlier games sometimes it didn't count collectibles (leaving you short, no matter how many times you went back around). And then ofc unity was supposed to be really bad, didn't experience any of the bugs personally but it was kind of renowned for it.

    I don't have sleep darts(not sure if they even exist in this version of ac) and anyway I found any action towards them other than creeping up behind and knocking them out resulted in you being seen. When that happens Karl Marx gets shot and you are at the start again. They clearly weren't meant to be stood looking at each other because eventually when it got down to the last two they ran off and i got them easily then.
    I got a glitch today, had to chase down a thief but they just froze and I couldn't do anything to them.
