Directions questionnaire confusion

Hi guys,
I am at the stage of the Directions Questionnaire after following the process to appeal my ticket. I am very confused about what I should do from here on out. I have followed the noob post but for some reason I have received a completed Directions Questionnaire from Parking Control Management (Uk) Limited. Is this a mistake? There are no instructions on the Noob post to explain what happens next.
I would also like to add that I came back from holiday today and received the letter with the directions questionairre, I'm presuming this was sent to my house two weeks ago. I want to make sure that this matter is not time sensitive.
