followed the link to a very dodgy looking web address - just a plain form, no branding and no corresponding website. I'm suspicious. where did you find this?

It's always good to be cautious but this is from an email from Universal Music, a classical and jazz label. The form does look basic but I've no reason to doubt it.

The 'updates from Blake' they refer to are from a 4 piece operatic group.
