My safe foods consist of tuna , broccoli , light mayonnaise , cabbage , and stock , light and fre yogurts and there's one that I allow myself more for preventing hypoglycaemia and that's an apple mullur rice ... all of this is purged straight after eating . Physically I suffer fatigue , dizziness , non diabetic hypoglycaemia, headaches , stomach cramps , fainting , if I bend over I instantly vomit , laryngospasm ( choking on air ) , tooth abscesses , rigidness around routine , and depression ( which holds its own physical symptoms )
Your points in bold should get you points if you explain this in the box for activity 2 Nutrition. That's a really good explanation of why you need assistance / prompting for activity 2.
There is not an activity for cleaning, so to score the necessary additional points you need to bring this compulsion into the descriptors on the PIP form. (as you do when you say "If I do eat then I have to purge because I don't deserve that food as the house isn't clean "). So, for instance does it take you a long time to wash / shower because the bathroom has to be clean before you start (and after)? This should score you 2 points (needing prompting) on activity 4.
What problems do you have dressing?
The bit I have italicised could help in other descriptors - i.e. because of dizziness do you need to sit when preparing a meal to be safe? If you need to use a perching stool - then this is an aid and should score you 2 points on activity 1. (see https://www.benefitsandwork.co.uk/personal-independence-payment-pip/pip-points-system)
As I said in my previous post - the form needs to explain how your conditions fit into the relevant PIP descriptors and points system . PIP can only be awarded on the basis of these points not your conditions.
Remember you need the PIP2 form to explain why 8 (at least) Daily Living points should be awarded.
Because this PIP form is important for you, please try to get help with it.

The ESA decision can take time, but the report should be with DWP now. You can ring up and request a copy, this should give you an indication of how the decision will go.0
