Lloyds PPI - no information

Hi.
I'm hoping somebody can offer some advice.
A while ago I had several accounts and finance products with Lloyds tsb. When I got divorced my husband took all the paperwork.
Due to the recent press with PPI I filled out a short form on Lloyds website with my name, address etc advising I might of had PPI. Today I've received 3 letters with 3 different account numbers and PPI reference. The letters give me no information but ask me to fill out the questionnaire. This asks things such as the date the product was taken out, reason and how PPI was sold.
I don't know the answers to any of this and wondered of anybody could advise on how I proceed.

If you dont know then you state you do not know. As simple as that.
Be aware, that if your complaint is successful and its regarding a joint PPI then you will only get half the redress. Or you may be required to get in contact with the ex for them to contact the bank and agree the split between you.I am an Independent Financial Adviser (IFA). Just out of curiosity I filled in the same form online from Lloyds and also received the similar letter and questionnaires to which I had no idea of the answers.
I went to my local branch and asked for the details of the loans, the clerk pulled up all my details in a matter of seconds and told me what each of the loans were and when.
I used this information and sent off the forms. That was about 3 weeks ago, this morning I went onto my bank account which I do every day and found a deposit for over £5K for a miss sold PPI claim on my loans.

I no longer bank with them but will try, thanks.
On another note I made a complaint about a packaged bank account and got a cheque for 569.02 today.0
